I made this last night, it's a piano arrangement of Steve Vai's "Ballerina 12/24" from Passion & Warfare.
http://s92612683.onlinehome.us/tab/ballerina.pdf
Info behind the original performance: It's one clean guitar with 2 delayed/panned/pitch shifted voices. A major 6th delayed by 1 16th note (right channel), and a perfect 4th delayed by 1 8th note (left channel). For example, bar 5 LH starts with C, A, F+C. The only notes played on guitar are C, C 8th notes an octave apart, and the A and F are delay repeats of the first C. At bar 16 it's easier to see this, the note + delays are isolated per hand and it's an octave skipping type thing. There are a few spots where I left a delay repeat out or left a note that technically shouldn't be there for consistency, but it's basically 98% accurate to the original.
Since there are some sections that repeat except for having a different delay note coming from the bar before, I went with parentheses = 1st time only, cue size = 2nd time.
Mike Keneally did a CD of Vai songs arr. for solo piano, including this one. It hasn't shipped from vai.com yet but I'm anxious to see if he came up with something that sounds similar.

It's great, Jordan. Thank you very much!
Now I'd better give a listen to the song because I'm not sure how to read those crazy first bars.

These are the rest of the pdfs I have online right now. Some of them are short and possibly useless. :-)

midi of the last one because it sounds cool: http://s92612683.onlinehome.us/midi/PoS_of2beginnings.mid
The intro of Ballerina is moving a 3 note shape around and strumming through it slowly. So you have F#, B, F# actual notes with D#, G#, D# coming from the first harmonizer repeat and B, E, B after that. The big chords in parentheses are everything that ends up ringing if you want to arpeggiate up the whole thing, but the slurred stuff is the actual order of each one. Since he strums through some faster/slower and the delay is a set time, it doesn't always end up being the same thing.

That's right!
I made some small changes to the look of the Ballerina score. If you found it potentially useful, you may want to download from the same url again now.
